I just purchased a Gigabyte Z170x-Ultra Gaming motherboard.
For some reason, every now and then, the USB connections disconnect/reconnect.
    I have a USB 3 SSD, a USB 2/1 keyboard, and a USB 2/1 mouse.
I do a fair amount of work, from that hard drive, so I notice when it disappears.
My keyboard/mouse lights flash, as well.

    Specs:
    Gigabyte Ga-Z170x-Ultra Gaming Motherboard
    Intel i7-6700 (non-K) CPU
    2x8GB GSkill Ripjaws V DDR 2400 RAM
    Samsung SM951 Nvme 256GB SSD
    EVGA W1-500 PSU
    Zalman MS800 Case
    Windows 10 Pro 64-bit

Drivers seem to be all up-to-date.
I was thinking about installing the USB 3 drivers (Intel), but it looks like they support up to Windows 8.
Other Intel drivers do say Windows 10, so I wasn't sure if they were needed or if Windows Update drivers were better.

Does this seem like a software issue or a bum board?

Does that occur on all the USB ports or just the USB3 ports?

    Hard drive is rear usb3.
    Keyboard/mouse at rear usb2/1.
I guess I could try the front ports from the headers.

    JJ,

I've got the same board with an i7-6700k in it and I'm noticing USB issues, as well. Different symptoms than yours, unfortunately. In Safe Mode, I cannot get any USB mouse to work. I'm curious: do you have Safe Mode issues?

FWIW, I've loaded the Intel USB 3.0 drivers.

Hello... Are you using USB 3.1 devices? on your mail you mentioned you used Samsung SM951 NVME 256GB SSD?
Then you are using USB 3 SSD?
I suggest you use the latest Gigabyte website driver. If you are using USB 3.1 you need to install Thunderbolt driver for USB 3.1 driver.

    JJ,

Go to Computer Management, and expand the USB controllers/hub section and take a screenshot.

My external hard drive is connected to the USB 3 port not 3.1.
My understanding is the 3.1 port is fed from a different controller.
The Samsung is an M.2 card.
I did install the Thunderbolt Firmware buy I did not install the thunderbolt drivers.
I don't have any thunderbolt devices and I didn't have any unknown things in device manager.

    JJ,

Well, I'm at a loss. Here's my USB image...they are pretty close. I don't see anything obviously wrong.

Is there any pattern to when the drive dropouts occur? Maybe it has to do with high bandwidth loads hitting the M.2 SSD, causing the chipset to drop the external devices.

I've had the external ssd connected to the front USB 3 port, for a couple days.
I haven't noticed the drive disappearing or my keyboard/mouse turning off.
I'll try connecting the drive to some other ports.
Maybe it's just one bad port?

I moved the drive to a different rear USB 3 port and it disappeared twice.
I'm moving it to the rear USB 3.1 port, now.

After 6 hours in the usb 3.1 port, the external drive seems fine.
I think the problem may be with the rear 3.0 ports.

I used a different rear USB 3 port and it failed, as well.
The issue seems to only be with the rear USB 3 ports (front USB 3 and rear USB 3.1 seem fine).
I'll try installing the USB 3 drivers, just to see if that makes a difference.

    Hmmm...
There are two USB 3 drivers...

USB 3.0 Driver: Intel® USB 3.0 eXtensible Host Controller Driver for Intel® 8/9/100 Series and C220/C610 Chipset Family
Intel® USB 3.0 Host Controller Adaptation Driver for Windows 8.1*

The first one seems to be for Windows 7 and lower.
The second one seems to only be for Windows 8.1 and it says Adaptation Driver (not sure what the difference is).
    Recommendations?

Interestingly enough, both driver install files errored and said that it wasn't a supported operating system.
When I tried installing the drivers, through Device Manager, it said that the newest driver was already installed.
    So, no advancement there.

Well, the front USB 3 ports had a failure, as well.
Maybe all the USB 3 ports are iffy, but the USB 3.1 port has performed flawlessly.
Of course, it uses a different controller.
I don't really want to go back to USB 2 speed, but I could try the drive on those ports to see.

I had similar usb issues when I first built the Skylake rig in my sig, but subsequent firmware and driver updates fixed them right away and everything has worked perfectly since

    What kind of firmware?
I haven't really seen any firmware.
    Drivers, yes, but not firmware.

    Hello Double J,
I see your attachment of device manager, I can see that your USB 3.1 revision is still at 1.0.
I have checked my system device menager my USB 3.1 is revision 1.1 version. (Please check attached picture).
I suggest that you should update your Intel USB 3.1 version.
Eventhough your USB 3.1 is not the controller for USB 3.0, but I think there is an relation to your problem.
Please do also install your thunder driver as well.

    Hehehe...
    My 3.1 hub is 1.10.

I did install the thunderbolt drivers, though.

Thanks. Oddly, the firmware update runs (version numbers are correct, B16.0315.1, but it never gives me a restart requirement. It shows a blue progress bar flashing across a few times and then says it's done.

Maybe try to connect to internet while updating firmware to check.
Maybe you should submit a ticket to Gigabyte or send your board for repair. Thanks.

I'm not using an SSD adapter.
It's an external Seagate drive (see above screenshots for drive information).
I'm also not using an M.2 adapter.
The M.2 slot is on the motherboard and it's a Samsung SM951.
I don't think the drives are the issue.
The issue seems to be on the motherboard side.
The USB 3.1 port (not driven from the PCH) has ran flawlessly.
Every rear USB 3.0 port, that I've tried, has failed at one point or the other.
At this point, I don't recall if the front USB 3.0 ports have failed (I've moved the drive around many times, by now).

Here's a question that may be worth asking...
Let's say, theoretically, that one of the front USB 3.0 header pins broke when someone (again, theoretically) installed the front USB 3.0 cable.
Maybe, because of said theoretical situation, one of the front USB 3.0 ports doesn't work but the other does.
I wouldn't think that would cause any issue, with the rear ports.
    Would it?

My installs didn't match your flashtbt 4 and run flashfw screenshots, either.
Of course, maybe the difference is that I don't have any thunderbolt devices.
Also, I don't have a Firmware section on my Device Manager.

I can say that the front USB 3.0 ports have issues, as well.
So far, the USB 3.1 port is the only one that seems 100%.
Well, the USB 2.0 ports are fine, but who wants to put an external SSD on those?
