86C max isn't too bad considering the speed, voltage, and the fact that HT are on. What cooling are you using? Also, what programs were used to stress test it?
For the memory, you might want to try 1T command rate. If it doesn't take, you can try increasing the DRAM voltage slightly. The QPI voltage should be fine.
All your power-saving settings are off at the moment, which you may want to re-enable after you've stabilized your clocks. I'm sure you already know what you're doing there.

I was able to get my D0 920 to 4.0GHz with 1.2V, try bring the voltage down a hair, say maybe 1.35-1.38ish, it will surely lower the temps, if its not stable then just go back to the voltage ur at. If i had mine at 1.58 my core would melt my motherboard.
It depends on the CPU. My C0/C1 Core i7 920 requires 1.475v to run at 4.2GHz.
