Entirely gaming. It's going to get a lot of use. Hopefully it will last awhile!

2) What's your budget? Are tax and shipping included?

$1500 total, including shipping.

24" 1920x1080, maybe upgrade to a larger monitor down the road

9) What features do you need in a motherboard? RAID? Firewire? Crossfire or SLI support? USB 3.0? SATA 6Gb/s? eSATA? Onboard video (as a backup or main GPU)? UEFI? etc.

The latest SATA ports. Don't need RAID or firewire. SLI/crossfire support would be nice but not required. USB 3.0 for future expandability. Don't need onboard GPU. Onboard audio is required.

10) Do you already have a legit and reusable/transferable OS key/license? If yes, what OS? Is it 32bit or 64bit?

If the OP isn't overclocking, IMO, the third-party cooler isn't really needed. Though the ones we recommend perform better than the stock Intel HSF, many of them aren't exactly quiet.
