X1800XT Artifacts problem

I have a Connect3d x1800xt 256mb, been using it for about 6 months and recently noticed some really bad artifacts and video card crashing (sound and windows stays on but no signal to the monitor). First thought was overheating, but I checked it and the card never goes over about 60-70 max load with an arctic cooling accelero x2 on auto speed, I even tried having it on 100% all the time but to no avail except reducing the temperature by about 10C. I have also tried loads of different catalysts and omegas (with drivercleaner) and even a reformat.

I've also ran Prime95 and memtest which came up with no errors. NB. the artifacts never occur in windows except for sometimes slight cursor corruption, but the crashes do occasionally. I'd like to ask your advice before I RMA the card please, here's some screens:

Have you tested your card in a friend's system? What power supply do you have? Welcome to Hardforum; sorry that nobody posted on your thread for 20 hours. Hope you haven't gotten the bad impression. I used to own a X1800XT (Sapphire) and I loved it.

P.S. Please list out all of your system specs as well.
