Why is my videocard bugging out?

I've had this 9700 AIW for almost 2 years now. I just went into Counter-Strike and textures started going crazy. There's a screenshot below. After a minute, the screen just went black and the sound in my headphones turned to a screech. I shut down, went to boot up, and it said I had to connect the power cable to my 9700. I opened case up, tightened cables, and it booted up fine. I dont know if I better secured a cable, or it just had a moment to cool off. The SS shoudl give you a better idea.

I'm just running a huge Zalman passive heatsink on it. Heats up a bit, but my thermal probe by the core is reading 50*C... so that shouldn't cause major problems.
 
I just played for an hour without any problems, but side of case was off. Perhaps I'll put the active cooling back on.
