I bought a rack today for my home and at the top was this 24-port patch panel but I'm not familiar with the back of it. Anybody know what this is and/or what you connect to it?

It's an RJ21, yes. They're used to hook up huge numbers of serial terminals. I've seen that setup used with an old HP PA-RISC machine (had breakout boxes like the one you had that you connected all the terminals to).
 
FYI you can use those for computer networks as well. They make replacing entire core switches or modules a breeze. Just pop the big cables off the back of the patch panel and plug a new one in and bingo you just repatched dozens of hosts into a new switch without all the spaghetti mess.
 
RJ21 is the USOC code for a 25 pair phone network interface block. The connector is more commonly referred to as an Amphenol. It looks like the panel you have is wired for all 4 pairs to each jack, a lot of the panels were wired for 10BaseT.
