weird audio problem

Sometimes. Sound comes through the speakers. Sometimes through the headset.
The sound used to come through both at the same time.
Why is this weird?:

On YouTube some videos will play speakers only some headphones only.
When testing in windows both work at the same time.
For skype only the speakers work.

In a game I play I used to have the option for headphones. But now that is gone and it will only play via speakers and headphones at the same time.

I have restarted and looked at my options but have no clue what is going on here.

Any ideas?

Anything changed from when it used to work until now?

I'd start with manually reinstalling the audio drivers.
 
While playing an audio file, if you change your default output device, does the sound move to the newly assigned device? (right click on the speaker in your system tray, playback devices, select one and press 'set default')
