Hello

I am looking for some help on finding a simple and streamlined solution. I need to be able to enter in information on clients that i have visited, what kind of problem, and if it was resolved or not. I am looking for something myPHP or MySQL based.

It needs to be accessed by others with in the company by just entering a USER ID and password.

If you need me to explain in more detail please pm me if you think you can help or have a solution for me.

Yup. First google hit on "RT".

Ticket systems are pretty simple - this is why there's so many to chose from (many of them rather good). There's a lot of OSS ones out there, so you should be able to find one that is either exactly what you want, or close enough that you can change it simply.
 
trac is oriented toward software/project development, probably not much help for a helpdesk/issue tracker kind of thing
