I've never had to use VSync before Doom 3. I've been using it and reading many of the explanations on this forum which have been very informative. However, I did have a question.

My refresh is set for the default of 60 which is fine for me. I seem to get FPS of 60/40/30/20. The 60 and 30 make sense but should I be seeing FPS in game using com_showfps 1 of 40?

? with vsync on the frame rate clock should only be showing 30 or 60 (well depends on what the actual frame rate would have been if non v synced)

Same here...never used it before. It did smooth out my Doom 3 frames though.

I probably have to double check my refresh rate. Maybe I had it set to something else but I don't believe so. I thought this behavior was strange also.

I think the fact that there are very light areas adjacent to very dark areas with lots of contrast, the frame tearing is very noticeable. I would suggest using vsync too if you have the performance headroom.
