Vista - No option to create a text document?

Yep, it's a default option there. Did you happen to load another text editor and then remove it by any chance?
 
I didn't load anything, and I have no idea how it disappeared. I'm sure it was before, but now it's gone.
 
This almost always happens if you install a third party text editor.

My guess is the .txt extension isn't associated anymore...

This may be the quickest way to fix it- go here and run the TXT fix... http://www.dougknox.com/xp/file_assoc.htm Extract to your desktop, run it... see if it is fixes.
