video card crashes

I need some help guys. I have a sapphire 4850 with an accelero S1 cooler. I also have attached a 120 mm fan to this cooler. Im not overclocking this card anymore ,but at one time i had it at 710/1190. I have really gotten addicted to Company of Heroes and sometimes during some intense action my card crashes.i think its the card because after the crash i have major graphic issues. I have to restart to remove errors. I have the latest drivers for card and latest patches for game. when i first installed this cooling setup ,I was able to torture test this card with Furmark for over 10 minutes and the temp never rose above 50 celcius. Do you think the card is defective,The game has a bug, or I fried the card when I was overclocking it? Any suggestions would be greatly appreciated. The problem usually occurs after I have been playing for sometime.
 
Could be your power supply.

Have you tried reseating the video card?

Have you tried reseating the RAM?

How is dust buildup in your case?

RUN GPUZ to see what your other heat sensors are reading. The core may be cool but other temps may be too high.
