If you are a fan from a Visiontek GF3, about every 4 months.

Never lost a fan on any other cards, but it is by no means unheard of, it does happen.
 
It depends. You honestly could have one last for years........or not. The info is probably more readily available for aftermarket than oem fans. For example, the Vantec Iceberq4 Pro I just bought is rated for about 50,000 hrs, but it could be more or less depending on the abuse I give it. Check the maufacturers website.
