USB Device Not Recognized

Recently I've been having a problem and I have had no success trying to solve it.

Not too long ago my PC started to boot and operate very slowly every so often. A reboot usually fixed the problem.
But now this is happening pretty much every single time. The time it takes to start is slower and when it does it's like running through molasses.

I also started getting "USB Device Not Recognized" errors.

When I click on the error I usually get this:

Other times it does show something:

So here are the things I have tried:
Checked this guide: http://answers.microsoft.com/en-us/...-nothing/561cb859-876e-4c76-baf2-e5524cdc68c1

At first it seemed like the problem was caused by Motionjoy. A program which allows you to use a PS3 controller on a PC. I then completely removed Motionjoy from my PC, or at least as well as I could.
Then I suspected Antec Chill Control. Removed that as well, then noticed Antec doesn't have that program on their site anymore. This made me quite suspicious but I still managed to find a place to redownload and reinstall it.
I also tried just deleting everything under "Universal Serial Bus controllers" in the Device Manager but that didn't work either.

It feels like some USB device is constantly connecting and then disconnecting from my PC and I have no idea what it is.

I have a feeling it's not an external USB device either.
These are the external devices connected to my PC through USB:
Keyboard
Mouse
Microphone
and a pass-through connector going to my monitor.

Does anyone know how to solve this issue?

If you disconnect the monitor USB does the problem go away?

Just some troubleshooting steps:

Test the devices on another PC if possible, otherwise try one at a time.

Run Memtest or Prime95, rule out RAM

Check the HDD for SMART errors at least.

I have not tried that but I will try that next. Although the monitor has been connected this way since I built this PC and it has never been a problem so I doubt that's the cause of the issue.

I do not have another PC. I have checked the hard drives and there doesn't seem to be an issue.

But then I ran Prime95 and I instantly crashed.

I would like to point out that this CPU is overclocked but I have had no issues with it crashing. I ran a lot of prime95 tests trying to get the optimal balance and kept tweaking the voltage until it passed the test. This was quite a while ago and I have no idea why Prime95 instantly BSoDs my PC now.

BSOD indicates hardware issue, maybe bad memory, maybe processor deterioration. May or may not be causing USB issue.

Is this your set up?

CPU:
Intel Core i7 2600K (Overclocked to 4.5GHz)

CPU cooler:
Cooler Master Hyper 212 plus

Motherboard:
Asus P8Z68-V PRO

RAM:
Corsair Vengeance 16GB DDR3

Sound:
Asus Xonar DX

PSU:
Seasonic X-760

GPU:
Asus Radeon 6950 2GB (unlocked to 6970 specs)

Storage:
OCZ Vertex 3 SSD 120GB
Samsung Spinpoint F3 1TB
WD Caviar GreenPower WD20EARS 2TB

Enclosure:
Corsair Obsidian CC650DW-1

Monitor:
Dell U2311H

If so that overclock for 3 years could cause problems. What Bios version are you on? Are you still having crossfire problems?

I mentioned the monitor connection because it is not a pass through, it is a USB hub and they do go bad or need a driver update.

Some things have changed.

CPU Cooler:
Antec Khuler 920

GPU:
Asus Radeon 6950 2GB (unlocked to 6970 specs)
+
MSI 6970 Lightning

Getting the crossfire to run properly took some doing but they are mostly fine now.
So you think that old CPU overclock is no longer working?

The motherboard BIOS version is 3402 which, according to the ASUS website, is the second most recent version. I suppose I could update it to the latest version (3603) if necessary.

As for the monitor hub, I've never used it. I guess I could just remove that cable if its causing issues.

P.S. I also performed 1 full pass on memtest and the program found 0 errors.

It could be Prime 95, the current version is not the same as the one you used when you set the machine up.

With that said, The BSOD indicates a Hardware Abstraction Layer problem directly related to the kernel and you have to start somewhere. I would start by getting your CPU voltage/OC back to stock, and run Prime 95 if you still have problems start basic trouble shooting.

Well I just installed the 3603 motherboard BIOS so my overclock was effectively removed. Even my RAM went back to less-than-default 1333.
I guess I'll run Prime95 again and see what's up.

Definitely not looking forward to having to redo the entire CPU overclocking process though. I hate overclocking (to be specific: The benchmarking/testing process) so I generally do it only once at the start and then leave it alone.
