Noob question, but I always wondered if when one spoke using voice chat, that only your team hears it or if everyone on the server hears it.
 
Depends on the server settings. Some servers it is just your team, some it is everyone. Best thing to do is ask when you get on a server.
 
Depends on the server, as a few have alltalk enabled.

Beaten by seconds. It becomes apparent really quickly though, as games aren't silent for long.
 
Slightly on topic: I miss characters mouths moving when you used voice chat back in Half-Life based games (Team Fortress Classic, Counter-Strike, etc).
