hello

I need a device that will connect my telephone with my pc and will provide as many as possible of these:

- display incoming calls and the caller id to my pc monitor
- record calls
- keep records of all incoming/outcoming calls, time, duration, etc
- automatically forward specific calls to another number (my telephone has the ability to forward calls, but not automatically based to the caller)

any idea?

I don't know of a solution out of the box to do this. However, if you are willing to learn and experiment, then you should be able to get all of the features working with an IP phone (or SIP ATA) + open-source PBX software, like Asterisk, Elastix, PBX-in-a-Flash, FreePBX, Trixbox, etc. I know that the package called FOP2 will display the CallerID of incoming calls on your computer (it's a web page that reads the Asterisk CDR if I remember correctly). Not sure about recording, but call records and rules are built right into Asterisk.
 
If you are looking for a whole office solution, Fonality/Trixbox does this with their HUD (heads up display).
