Swiftech CPU block compatible with socket FM2 / AM3+?

Hey all, I currently have watercooling with a Swiftech CPU block (sorry I don't know the model, it just says Swiftech on top) cooling my q9400 on my socket 775 motherboard. I also have a socket 1156 backplate for this CPU block.

If I should choose to go AMD for a CPU upgrade, will I have to buy anything else to fit the CPU block to a FM2 or AM3+ motherboard or can I just attach it to the motherboard directly?
