Is the surface flat enough or if I went through the trouble of removing my IHS from the p4 should I lap the heatsink as well?

There are fine machined grooves on the bottom. you might want to lap if you remove the ihs.
 
yeah, if you lap the heatsink to a PERFECT shine, and remove the integrated heatspreader, you could get a whole 3 degrees under the right conditions, assuming part of the core doesnt come off with the heatspreader when you remove it.
 
3 degrees?

I lapped my sp-94 tonight, 400 grit, 800 grit (couldn't find any 600), 1000 grit, 1500 grit and 2000 grit, and I got a 6.5 degree drop in temps. :D Worth the 12 dollars I spent on sand paper and the hour and a half of lapping, if you ask me :)

So what are your temps now? What fan?
