Is 10.12 the only driver set you tried? I use a computer with a 5450 at work and haven't noticed any kind of lag. I believe the drivers being used are which ever one Microsoft is pushing on Windows Update, so probably 2-3 months old.
 
I agree the driver is the most likely culprit. Did you remove the old driver before installing the new? Also, you might need to use a registry cleaner to ensure that a all remnants of the old driver are removed, especially when switching between nVidia and ATI cards.

Did you try re-seating the card to make sure it was well set in the slot?
 
I experience a bit of lag in firefox on my laptop which has a 540v (4650), but I think it has do do with flash crashing in ff. It is really terrible :(
