Say I buy an 8800GTS 320 now for $250, and in December I want to upgrade to the mid high end G92 which has an MSRP of $400 but sells on newegg for $350; would I pay $150 or $100 to step up?

You'd have to go on the evga site, which would list the MSRP of whatever card you want to upgrade to. For example, I also have an evga 320 which I paid 280 for on newegg and got a 20$ mail-in rebate for (= 260$). Say for whatever reason I wanted to upgrade to the 640 edition which is listed MSRP on the evga site as 399$. So it would be 400 - 260 = 140$, but then there's also shipping which you must pay BOTH ways, so tack about another 25$.

I'd say that unless prices drop dramatically it's not *really* worth using the step-up program unless you're doing a MASSIVE upgrade. Otherwise eBay is your friend :D

I had originally purchased my 7900GT with the intention of Stepping Up. Once the 8800's were released though, EVGA themselves were price gouging. They were selling their own 8800's for $50+ over MSRP. I thought it was a pretty slimeball thing to do.... so I didn't Step Up just on principle alone.
