PSU Troubles

My system has been locking up and crashing repeatedly which leads me to believe my PSU is going bad. I checked the hardware monitor in my bios and this is what I came up with:

3.3v = 3.28v
5v = 4.99v
12v = 11.58v

I've never experienced any PSU trouble before so I thought I'd come here and see what experiences others have had. Appreciate any help on the matter.

Here's my PSU: Aspire 500w
 
If the readings are accurate, the first thing that stands out is the 12V rail is WAY TOO LOW.

Double check the connection to the motherboard. If that looks good, get a multimeter and check the voltages again.
