So I haven't done any gaming in a few months nor any tinkering so this issue has me lost. At random, the screen will freeze (with the sound still going but frozen) for about 5-6 seconds, then both monitors will go blank (with the sound still going), and several seconds later the desktop will come up with the game still running but minimized. At that point I get a warning that states "VPU Recover has reset your graphics accelerator as it was no longer responding to graphics driver commands." I have removed the old driver and updated...still does it. Anyone?
 
You probably still have a corrupted part of the driver. You should uninstall the driver, restart your computer and then run Driver Cleaner. This will remove all the components that might have been left over by the uninstall.

Then download and install the newest driver you got.

This may solve the issue.
 
Just wanted to chime in here.

I have been experiencing the exact same issues with my X850XT...so has a friend of mine, but with a pair of 8800GTS's. I originally thought that the card was dying, but listen to this...

My friend and I have experienced the exact same issues and have completely different motherboard chipsets, memory and video cards...he has swapped out (at one point or another) nearly everything in his system and it still has not been completely rectified. The ONLY thing our systems have in common is the Intel Core 2 architecture. I noticed that you're running an E6400...I have an E4300, and my friend is running a QX6700...all Intel Core 2 based chips. Between the two of us we have done enough hardware troubleshooting to bring it to the point where it almost HAS to the our CPU's...and there was some information published regarding Core 2 architecture errata that could be causing all of our issues. Many problems seem to be fixed with the latest G0 C2D's...and many of the issues are very rare to begin with...but I just wanted to let you know that it might be your CPU as well.

I'm gonna dig up the article and gather my thoughts to start a new thread on the topic...because between my friend and I the issue has occured not only with nvidia and ati video cards...but over i965, 680i, and RD600 based motherboards with different memory configurations.

Watch the main video card forum in the next few hours and hopefully I'll have something to post.

EDIT: I just found a microcode update from microsoft that is supposed to fix a lot of the issues linked to possible Core 2 errata...and I'm installing it as we speak. I'll chime in later with results...hopefully all goes well.
