Problem with A64 3000+ Overclocking

Memory: OCZ 3200 EL Platinum rev 2
Cooler XP-90 plus Delta 92mm
PSU: Tagan 480U01
VGA: XFX 6600GT PCIE

Does anybody here got problem like mine?
I cant get overclock my CPU pass 256 HTT @ dual channels,
I did tried memtest 9x300 single channel and has no problem with it prime 95 stable 25 hours and 10 loops of test#5 each sticks.
same thing when im using OCZ VX 3200, i can get 9x255 without no problem.
The SmartGuardian read all the voltage normally but teh 12V fluctuate between 11.87v to 11.94v
The problem is everytime boot to windows the computer reboot automatically.
also i can use divider at dram/fsb 133 at 300HTT without problem.

Does it mean i have "bad memory controller" or i need to replace my tagan?

your voltage seems fine, its pretty stable...

whats the coltage running to your ram and your cpu?

Ram voltage for OCZ 3200 Platinum rev 2.7v
OCZ VX 3.5v

CPU 1.6v at 9x300 single channel
