I'm considering toying with the power saving cpu options in the bios, to get a bit more lifetime out of my cpu. Modifying them and running a prime95 test for a while isn't a problem, but since it's power saving stuff could I run into stability issues when not maxing my system?

Like C1E which I believe changes voltage based on load(?), is this something that's likely to work perfectly during stress testing, but crash under normal pc usage?

If you can pass a 24-48 hour prime95 stress test then your pc is considered stable.

PS: Your CPU will be outdated way before it dies so don't do this to extend the lifetime of your CPU. Do this only if you want to save power or lower your CPU temperature.
