PC repair disclaimer example?

im going to try and fix someones computer. his computer is really hosed, I don't really know him personally, but hes going to pay me, so I'm wondering if any of you know of an example disclaimer which works well with PC related things.

for example if his harddrive burns out while im sitting in the chair to his computer, so that he doesn't hold me responsible. i just want to cover my ass here legally, and i thought a disclaimer would be the best thing. ive been searching online for a long time but can't find an example. maybe someone here with a PC business might have one i could take a few things from?
 
"I hearby authorize repair work to be done using necessary metarials. *insert name here* will not be held responsible for loss or damage to equipment in case of fire, theft, accident or any other cause beyond your control. I understand that I am responsible for backing up any data before my computer is worked on. *insert name here* is not responsible for any loss of data that may occur during his work.

1. DISCLAIMER

1.1 NAME offers computer service, repair and upgrading services as is. We believe business should be conducted honestly, fairly and be subject to reasonableness. We strive to provide the highest quality of service and support. We cannot guarantee specific results for our services, but will operate under the principles stated above and expect you to do the same.


2. BILLING TERMS

2.1 Computer servicing and repair services are billed at an hourly rate. Charges will be calculated in quarter-hour increments, rounded up by default, and carry a minimum half-hour charge. We may, on a case-by-case basis, choose to round down to the nearest quarter-hour.

2.2 We will provide you with an estimate of cost for the work that you request us to do. Estimates are guaranteed to 15% accuracy. In the case that we foresee a deviation on cost of job completion beyond the above stated allowable amount, we will make very effort to contact you and inform you of the situation and receive authorization to continue or stop at the estimate limit. In the case we cannot reach you, we will continue to work until the estimate limit is reached. Once reached, we will halt work until contact is established.


3. PAYMENT TERMS

3.1 Full payment is due upon completion of servicing, upgrading or repair. Failure to pay may result in the debt being turned over to a collections agency and reporting to the credit bureau. You will be responsible for all costs incurred by us, collection agencies, or courts incurred by litigation for failure to pay.


4. LIABILITY

4.1 We provide our services in en effort to fix, upgrade, or otherwise repair the computer systems for which you request such services. We will not intentionally harm your system. It is our goal we want to fix your computer, not damage it more. In the case of accidental damage or data to your system, further damage or data loss caused by already existing problems in your system such as viruses, misconfigured software, or hardware problems/failures, you agree to hold us, NAME, and any persons associated with or involved in the work being done for you harmless from damages resulting from such problems.


5. SUPPORT

5.1 Customer satisfaction is of utmost importance to us. We will backup our work with support services. Free support will be provided for problems requested to be resolved in the work order but not resolved. Additional support may be provided free of charge at our discretion.
