Missing outputs on video card

I have a really weird issue:

My card is a Sapphire HD5670 Ultimate (link).
I have 3 monitors, Dell 2408WFP and 2x Dell U2311H. All were connected to HDMI (2408WFP), DVI (U2311H #1), DisplayPort (U2311H #2). None are using any converter cables, they are all homogeneous cables. They were working when I picked up the card last August and working until now.
I'm running Windows 7 Professional 64-bit.
The HDMI and DVI outputs are missing. I don't see them in AMD's Catalyst Control Panel. I don't see them in Windows 7's screen resolution menu.
The motherboard I'm testing it on is a ASUS P5N32-E SLI Plus. I only have one video card.
I tried this on another motherboard (ZOTAC NM10-BE), same issue.
The cables are not dead, the DVI cable works with another video card on another computer. This also means that the monitor is working as well.
Same with HDMI, I connected a PS3 to the 2408WFP with the same cable and it works.

What could possibly be the problem? RMA is an option but it's $60 or so and I'd rather put that money toward a 6000 series card with DP 1.2 (so I can use a MST hub and not worry about having a card with 3 digital outputs).

Any help would be great. Thanks.
 
If you connect the monitors individually, do you see a picture during BIOS POST?
 
The BIOS shows nothing with the DVI or HDMI cables separately. It is only when DP is connect I see something.

Also tried an alternate installation of Windows 7 on a spare drive as well as some Linux distros. Neither output to DVI/HDMI.

To add, the motherboard has no dedicated video either and primary output is set to PCI-E. No hardware changes were made from when I had 3 outputs to 1 output.
 
Did you try:
Re-seating the card?
Unplugging, replugging the monitors?
Trying the monitors on the DP?

This sounds more like a physical problem then a software problem.
 
I moved the card onto another motherboard and thus had to reconnect displays as well as re-seat the card. Catalyst doesn't show the outputs, it only shows 1 output which is DP.

I even cleaned the PCI-E contacts in case there was any problems with that in the odd case that oil from my fingers got into the slot and came up after 1 year.

The monitors are all fine, all DVI/DP/VGA are working on the 2 U2311H's and everything's working on the 2408WFP.

From the looks of it, it appears to be a hardware issue but I am looking for suggestions to see if anything could be fixed without having to RMA the card.
