I like a single video card that can max out every game (even Crysis) on a resolution of 1680 x1050 and with frame rate of over 60.

People often get high end cards (or even do SLI/crossfire) for high resolution monitor but for me what is important is being able to turn every features on max (max AA, etc...) but getting excellent frame rate at only a resolution of 1680 x 1050.

When the high resolution is taken out of the equation, what would be the quietest, single, smallest form factor card that I can get?

O.K. how about if you take out the Crysis games.

In most games you should be able to max out all settings with at least 60 frames at 1680 x 1050 with either a GTX 295 or 4870 x2 as a single card solution

And if price is also a concern, the 4870 x2 should give you above 60 frames for almost $200 less than the GTX 295, even though the overall highest frame rate may go to the 295.

Single card (well technically only the 4870x2 is) but still SLi/CF.

I vote GTX 285 or Radeon 4890. So maybe you'll have to bump down from 8X to 4X or 2X AA in some games... does that REALLY matter?
