I have an old Thinkpad X200, it will power up to desktop but as soon as you click on any icon or try to open anything it completes dies, black screen, and will not power back on without first removing the battery.

I believe this is a hardware issue as the laptop has a rough life in a dusty environment.

Any advise greatly appreciated.

Have you tried installing a fresh copy of Windows on it? This will rule out software problems.

What happens if you turn the laptop on and let it sit at the desktop? Will it sit there "forever" or will it eventually die and shut itself off? These questions are to eliminate over heating issues.

Have you tried running any kind of memory checking software to see if the memory is failing? Does it have more than one physical stick? Try using one at a time?

Yeah I tried one RAM stick at a time and it still did it.

Will try reformatting next. If that doesn't work, oh well, I'll buy another refurbished laptop for work.

Try running it on a Linux live distro first, will be easier than reinstalling. But yes, good to rule out software first :)

I suspect it is overheating though, I had an HP/Compaq that was doing this and it turned out the fan had failed. Replaced and everything worked great afterwards.
