Issue with PNY GTX 480

im having issues with this pny gtx 480 i got recently.
i dont know if its a heat issue or something else but the card likes to freeze up, occasional BSOD, and driver stopped functioning.

ive updated the bios to the videocard in hopes of helping it or even fixing it.

can anyone help me figure out whats actually wrong or if its even salvagable?
 
cards probably going bad.. theres been a lot of GTX 480's dying lately so my guess is its heat related specific to the VRM's.
 
So when you say VRM do you mean the volage or the videoram on it?
I took I apart today to see how it looks and replaced all the old thermal with rosewill nano silver paste. I didn't see any burns or anything but the old stuff was pretty caked on.
Maybe a fc h2o block would help it? Not too sure.
