Is there a difference in Language Pack or os in the certain language?

I am wondering is there a difference between installing a

Chinese windows 7 or a english windows 7 with a chinese language pack.

I have the Japanese language pack installed for my wife, and it is English Windows with support for Japanese characters and input methods and programs, but the OS itself is in English - I assume the Japanese/Chinese version of the OS would be fully in the other language - so instead of Control Panel it would have the correct words in the other language/characters. Never tried the Japanese version though, so I can't say for sure.
