How to keep FaH at highest priority?

FahCore_a7.exe is the 32-bit executable that runs the algorithm. By default, it's set to run as a low-priority task, at least with Win10 x64 Home. I use my computer maybe 25% of the day or less. The rest of the time, I want it to use every bit of power my computer has to run various FaH. I can manually set FahCore_a7.exe to "Realtime" in the task manager, but after every WU is complete, it resets back to "Low." There's got to be a registry setting or a script I can run to toggle Realtime on when I'm not using it, then either pause it or set it to Low when I'm not using it.

Have you seen an increase in PPD manually changing it? If so, how much?

Yes. It varies. When it's set to low priority, I get maybe 3000 PPD. When it's set to Realtime, and I'm home all day, setting it back to Realtime every couple of hours, I get upwards of 7,000. Back before nVidia's CUDA drivers for my 780 Ti were broken in a driver update (and on a platform way, way less powerful than my current build) I could get upwards of 16,000 PPD.

That's quite odd. I'm getting almost 90k PPD on my 750 Ti, which should be a fraction of what a 780 Ti gets. I do have a driver collection that I specifically use for 900 series and older cards for F@H, but I don't remember seeing a reduction in points quite that bad from newer drivers. You should be getting around 200,000 PPD on a 780 Ti according to online sources.
