How does V-sync work

I know that v-sync keeps the refresh rate and fps in check and that if you have too many fps for your monitor, you get real bad tearing.
But what I was wondering is how it does this, some people say if your refresh is 85 then vsync caps your fps so it never goes over this and some say if it gets near 85 it halfs the fps.
I really dont know myself, the only game I use it in is far cry and it really messes with my performance, time for a new monitor :D
Any insight into vsync from our more techy forumers would be cool.

Vsync caps the screen output so that the image is stuck at a limit, example 75 hz, 85 hz, which translates into an fps limit. Vsync alone, will cap your fps and limit performance. This is where Triple Buffer comes in to play. Triple Buffer allows the fps to be pushed to the 75 or 85 hz with a full 75 or 85 fps, as if Vsync was disabled. Yet, no tearing. :) Another thing to watch for with vsync is, when it's enabled and in certain sections of the game the fps drop, you'll notice a stuttering or "dragging" look I can't explain good in text, but its there.
