When do you do it? When it looks low... every few months?

How do you do it? top it off with still water?

Generally yes, it's good to top off things when they look low :p ;)
But in a closed-loop system, it takes *quite* a while to see any visable drop in water level. By the time you have lost enough water to be noticeable, it's probably time to flush and refill anyway :)

Plus, depending on the pump you use, if you can hear it when you power on your machine, it might be time to re-fill. ;) Seriously, just check your fluid levels once a month by eyeballing the res. If its a closed loop without a res, it becomes a bit more difficult. If you seem to have a lot of air bubbles in the tubing on startup, that would be an indication that its time to bleed and top off.
