How concerned should I be?

Reallocated sectors by itself isn't a big deal as long as it's not growing. But the current pending sectors count and uncorrectable errors counts are definitely something I would be concerned about. At the very least, I'd make sure any important data is backed up.

Here is what HD Sentinel says: There are 113 bad sectors on the disk surface. The contents of these sectors were moved to the spare area.
The drive found 43 bad sectors during its self test.
Based on the number of remapping operations, the health of the disk was decreased in different steps.
There are 213 weak sectors found on the disk surface. They may be remapped any time in the later use of the disk.
It is recommended to examine the log of the disk regularly. All new problems found will be logged there.

It is recommended to backup immediately to prevent data loss.

*Estimated remaining lifetime 5 Days*

I already bought a 2TB USB 3.0 HDD to back up the data but it is just game files anyway. Just don't want to have to download all my Steam and Origin games again so will back them up to this HDD. Guess I better get a new HDD soon even though this one is still under warranty, but WD will just say it hasn't failed yet. There is a Dos utility on Hirens Boot CD that tries to repair damaged HDDs but it takes a very long time, think it is worth a try?
