hard drive error rate/not ready

I have a brand new HGST SAS drive that is not coming online after running a few verify cycles via the HBA and has the occasional chirp. I get over 250 hard errors in a hour. Any thoughts?

I'm of the opinion that if you're seeing hard errors this early into the lifetime of the drive, RMA it and swap it out for a replacement.
 
Try another Power/SAS Cable or slot, and if no change then RMA the drive it's toast.
 
I got 3 more HGST hard drives in. All drives sound good. In the SAS BHA menu, I did 2 verify cycles and a format on the first drive, just like the previous failed drive. They all passed. I then attempted to do one more verify cycle of the the format, but it failed. What could be causing the issue?

The drives are connected via:
Supermicro X8DT3-F MB
LSI 9207 SAS HBA, on PCI port 5
Supermicro BPN-SAS-846EL2 backplane.

Edit: It appears that somehow formatting the drives is killing them as I am now 3 for 3.
