Hard Drive constantly seeking

I have a problem. I just got my main PC back last night and my HDD is now constantly active/searching. It never used to be, and it never used to freeze for 10-20 seconds randomly, whether in game or on the desktop. Is my hard drive dead, or is something else going on?
 
Well, a little more specs would help
what do you mean by got your main pc back?

First, try checking the cables, then use a disk diagnostic utility, like SeaTools or HD Tune to check the integrity of your hard disk.

Finally, backup your data (I think this should be the first step :p)
