eVGA Mail-In Rebate?

Awhile ago on Newegg.com they had a mail-in rebate on the eVGA 6800 Ultra 256MB DDR.....I believe it was for $30 or $50....does anyone by chance have a copy of the actual rebate saved? Was an Acrobat file I believe.
 
I just purchased that card also. Does anyone know the time period that the card had to be purchased in?
