Error with video and audio

This has appeared a few times at random occasions, all of them today. It appears to be impossible to recreate. What happens is it freezes up, and the screen does what is shown above, and the speakers make a sound like a jackhammer.

AMD 2400+ @ stock
MSI KT4 Ultra
Sapphire R9000P 64Mb @ stock
128Mb DDR2100 RAM
39Gb Quantum Fireball HDD
CD-RW
DVD
Onboard sound
Windows XP Professional SP1
CPU temperature is ~90F, the case ~93F.
System is ~2 weeks old.

The case only has 3 fans in total: 1 case, 1 PSU, and 1 on the CPU. Could overheating be the problem?

The system only has 128Mb of RAM, could this cause an error that results in the anomaly detailed above?

I would say something wrong with your video card. Everytime I see that screen it usely comes from a failing Video Card.

most likely a vid card problem - if possible - try running this setup with another vid card just to see if you'll experience similar problems...
