Do you do fresh installs of iOS or is it a waste of time?

No - never had the slowdowns or instability typical of MS Windows if not reinstalled regularly occur on iOS or on OS X.
 
I do for troubleshooting. With how much iCloud syncs these days you end up losing out on so little anymore.
I just recently did it a few months ago when iCloud message sync was enabled. Didn't solve my issue, but was easy to do.

Last time I installed Windows on my main machines was back in 2009.

I usually do a DFU restore every major iOS release, but I run the betas so my situation is probably atypical. I'll probably be removing the beta profile off my phone in the next few days in anticipation of iOS 12 going GM. I made the mistake of leaving the profile on my phone and accidentally upgraded to the beta of a .1 release and was stuck on that beta for a while.
