Actually, the name is just for the application/protocol name using that port range. The OP needs to enter his IP address of his computer so his router knows which machine to forward ports to.
you should have an internal IP address in there... 192.168.x.y
it doesn't matter if anybody knows that internal IP address, since it's internal, it's non-routable, and nobody can do anything with that info.
what IP address are you entering in that box? that would help greatly.
and you do have a static IP set on your computer, correct? or, at the very least, setup a static DHCP entry? I believe the DLink supports that, but I could be wrong.
