Anyone here know how I can get a custom case made and in quantity? What kind of shop or people would I approach and how much does it typically run?

Locally, you could hit up a metal fabrication shop, machine shop, or prototyping shop. Check your phonebook. I don't know of any online sources offhand, except for http://www.mnpctech.com. How much does it run? It could be anywhere from $20/hour to much more, depending on your location, labor time per case, who provides materials, how much CAD or sketch homework you can provide up front, etc.
 
Maybe, but it would be far from practical, given that they are an overseas outfit and produce cases in relatively high quantity. If you intend for your case design to be sold in comparable volumes, then I'm sure a company like Lian Li would at least entertain your proposal.
