While playing a game (EQ) my 9500 pro seems to lockup/crash and the video signal seems to 'die'. The monitor goes black, but the system is still running, I can do ctrl + alt +del and hit T to bring up task manager, and they HDD LED shows activity, so I know the OS is still fine.
Using ATI tool I scanned for artifacts and found none. I am running at default speeds. I bumped both the core and mem down about 6 MHz and it still crashed. Sometimes it will play the game fine for hours with no issues, but it locked up on me twice last night.
I decided to run 3D Mark 2001 to see if it would crash during any of the tests since it seemed to crash at some graphically intense parts of the game. I noticed that during the Pixel Shader test the card would lockup right away in the first few frames. This happened repeatedly in 3D Mark on the same test.
I searched the web and [H]F and didn't see any other issue like this.

MSI KT6V mobo
Corsair RAM - 512MB, 400MHz
Athlon TBird (wooo!) 1.4GHz, SK7, 70mm TMD fan, temps at 62* during gaming
9500 Pro w/Vantec Iceberq, rim spacer removed
Lian Li case w/stock fans
Win2k Pro SP4
Catalyst version: 4.10
DirectX: 9.0b
This is a fresh install of Win2k too, nothing on the machine but the game and utilities i mentioned.
