Cheapest Graphics card that does 4K output

Anything modern really. Id go GTX660 or higher but you have other choices.
 
According to my specs the GT 640 and GT 730 are the most basic cards that support 4k 24Hz via HDMI. If you're wanting the full 60Hz you'll need a GTX 960 or better for HDMI 2.0 support.
