In case anyone is interested in playing this one again. I was able to get it working under Windows 7 64bit @ 1920x1080
Here is how.
Install the game, then download the Super Patch V2

IMPORTANT
Now that you have the super patch extracted to your Carmageddon 2 directory, Only use the CARMA2_HW.exe to run the game. Also right click on the .exe and go to properties to select Windows 98/ME Compatibility mode
After installing the patch (and making a shortcut of CARMA2_HW on your desktop) you want to go here and get Nglide Wrapper

Nglide wrapper allows you to play all of your old Glide / 3dfx games at widescreen resolutions.
After its installed, run the Configure icon to set your resolution.
As for controls , I was having a problem with the game not letting me use the directional D-pad for movement on my Logitech Dual Action II, so I did a bit of research and found this simple program to emulate my controller to match my keyboard.

After all that, it should be working for you, the only thing that isn't working is the in game music, this is a problem with the Super Patch V2 I hear. (No Iron Maiden
) But the game works flawlessly and there is always Winamp!
