Building a new computer. Suggestions please

My current computer is starting to suck so I thought I would get some advice here. Thanks in advance.

Edits- removed PSU and SSD from parts needed.

1) What will you be doing with this PC? Gaming? Photoshop? Web browsing? etc
The computer will be used for gaming. I was looking at mini-itx for the potential to transport it. But I feel like the price would probably drop without going that route. Opinions would be great.

2) What's your budget? Are tax and shipping included?
Hopefully 700-750 but maybe less? let me know what you think.

3) Which country do you live in? If the U.S, please tell us the state and city if possible.
I live in Minneapolis, MN.

4) What exact parts do you need for that budget? CPU, RAM, case, etc. The word "Everything" is not a valid answer. Please list out all the parts you'll need.
CPU, ram, mobo, video card (at least 2 gb).

5) If reusing any parts, what parts will you be reusing? Please be especially specific about the power supply. List make and model.
Mayyyyyybe could reuse a decent power supply I have but I want to use it for a second media computer.

6) Will you be overclocking?
Nope.

7) What is the max resolution of your monitor? What size is it?
1920x1080, 23"

8) When do you plan on building/buying the PC?
Within the next month or two.

9) What features do you need in a motherboard? RAID? Firewire? Crossfire or SLI support? USB 3.0? SATA 6Gb/s? eSATA? Onboard video (as a backup or main GPU)? UEFI? etc.

10) Do you already have a legit and reusable/transferable OS key/license? If so, what OS? Is it 32bit or 64bit?

This whole build is almost 750USD.
Now, IF I could invest a bit more, I would double the RAM for 30USD, get the 4GB version of the GPU for 20USD, and add a small SSD to use the M.2 slot as a drive accelerator.

Here's something quick I came up with. You don't list a case as needed, but you did say you would prefer a small build and also would prefer another PSU, so I put this together based on that. The only thing is, I hope you already have an SSD. If not, this could use more reconfiguring.
