Been having BSOD and its happened twice now, could be the mobo (asrock) or I have no idea!
*** STOP: 0x000000F4 (0x0000000000000003, 0xFFFFFA800B9848B0, 0XFFFFF80003B9B8B0)

First off my computer was running FLAWLESSY up until I started crashing while playing FO: NV with a 6950 gpu.
This was the flawless setup:
i7 965 es
corsair push>pull h60
asrock extreme6 x58
vertex 2 120gb
2tb caviar green
600gb velociraptor
12gb vengeance 1600
gtx 480 and later gts 250
antec 850w tpq

I then switched out the gts 250 to handle FO: NV with an MSI 6950 twin frozr II 2gb OC, started crashing with amd driver crashes and never loading. While I was waiting for a 6950 replacement, I had the GTS 250 in running everything ok, just not on ultra settings for gaming.
Replaced the ssd with a Patriot Wildfire 120gb SSD, reinstalled windows 7 64 Home.
Got a GTX 580 1.5gb (which ended up crashing and giving me lines across the screen), RMA'd that. During that process the EVGA tech asked if it could be the PSU, replaced that with Antec and now have a 850w HCP.
Got another 580 again this time a 3GB and running the rig in my sig...

Ever since the 2 BSOD's which I have a feeling have the same error numbers the mobo code would be stuck on 75 which meant harddrive. For the 2nd BSOD I moved every connected sata cable to a vacant one. Running my rig right now as I'm typing this.

If anyone can help me I would greatly appreciate it! I have a free batman sinestro skin for Arkham City (ps3 code). LOL sorry thats all I can offer since I'm broke and currently hit tough times.

Done any stability testing? Is your ram 3x4gb or 6x2gb?
Memtest/linx/furmark?

what have you tested so far? the troubleshooting guide sticky has info on what to test and how.

EVGA tech support asked me to run memtest 86, it passed 100% with 0 errors.
Havent used furmark.
My mem is three sticks of 4gb = 12gb, they're in the correct spots.

Thats it as far as tests go, thanks a lot for the help I GREATLY appreciated it!

I was having a similar issue, a recent firmware update for my Vertex SSD solved it. Mine was for the Vertex 3 series, not sure if the Vertex 2 had the same issue.

I just ran Furmark like a motherfucking noob: Ran BURN-IN test instead of benchmark.
1920x1080
4x MSAA
[x]Dynamic Background
[x]Burn-In
Ran for 15 mins, avg 9fps highest GPU temp was 64c.

Im going to run bench when I cold start my computer again tomorrow.
So far ever since I switched the sata ports on all the drives everything seems to be fine.

I read some asrock x58 extreme6 reviews in newegg that these sata ports were first go and they would RMA the board.

Also every time I check HW monitor in asrock bios its reading 12.936 for 12v rail. I guess the reading is normally high because my Antec TPQ was reading at 12.830.

If everything is fine, then don't worry about it. The burn-in test will actually test stability, so run that for a while if you can (however long your gaming sessions usually are would be a good length of time).
