Pretty much a noob when it comes to OS issues, but i've been getting random blue screens on an old rig for the past month

I've replaced ram, fresh os install..even swapped in a new HD

Here's what i got from bluescreen view.

I got "Bad Pool Header" BSODs when I was overclocking an older system. It was a Phenom II X4 and I was pushing both the CPU and IMC. Finding the right voltages for the speeds I was running got rid of those completely. Try turning the voltage up a step on the CPU and see if that helps.
 
BAD_POOL_HEADER generally indicates a memory problem, as it indicates the memory in a pool header has been corrupted somehow. This *could* be a S/W issue, but the majority of the time I see this BSOD pop up, the offender is bad RAM.

The first obvious test (after you validate your RAM voltages/timings) would be to run memtest86 for a few hours and see if the RAM reports any errors.
