AV-710 front panel help

How the hell does one connect front panel connectors on my noname brand case to this thing. I have a diagram that I found on Google, but damn, nothing works.

Here's the diagram.

My case has 2 of each of the following cables: Mic In, L In, L Out, R In, R Out, Ground.

From what I can tell, I have no clue where to plug the other Mic In and the outher Ground cable...and with everything else plugged to the pins according to the diagram, the front panel audio connectors do not work at all(no sound, no mic, no nothing, etc).

Also, what the hell do the 2 notes mean on the diagram?
