What are your PC's specs? Which CPU, the amount of RAM, Windows version?
Intel Core 2 Duo E8400 3.00GHz
4GB DDR3 RAM
Western Digital WD5000AAKS
Windows 7 Ultimate 64-bit
I'm going to try hardware diagnostics, I remember the machine completely froze while calculating the WEI score, so something is not working right under full load.

1- are all your drivers 64bit?
2- I built a rig for a friend and everything was slow. Turned out that BIOS had defaulted to a lower FSB because it did not recognize the CPU.
3- check out the hard drive. is the drive activity light always on? if it is, then you may have too many things running in the background and windows is using a lot of virtual memory (page file)

I don't know that your problem is the video card but yes the 3450 does do HD, though I would be getting a newer card myself.
All drivers are 64-bit. I installed the official Dell drivers and it had no effect. Nothing else runs slow.
Will run diagnostics during thanksgiving break and see what turns up, this is really odd. I saw computers with worse specs handle HD just fine.

your specs are fine, it's not a hardware issue. My guess is it's a 64bit bug somewhere, the fun part is finding it. My advice is venture down into the HTPC section of the forum. Should get some straight answers there.
