Hi guys,

The title may not be correct - it's just what I perceive to be happening. My goal is to replicate what is going on in this form with these buttons: http://www.montclair.edu/ORSP/irb/documents/irbapplication.pdf

Aside from the button, I would also like to replicate what is going on on page 2 (item 5).

What I do know: I know how to create basic forms, and I know how to create buttons. Will I need advanced Javascript functionality (which I don't know) to accomplish the tasks that is going on in this example form? It seems like more advanced things are going on here other than simple hiding and unhiding - the document moves and new fields are added (within limit).

Any help is greatly appreciated. Thanks in advance!
 
Just a tip, I'd PM a mod and have them move this to the Webmastering and Programming area.
