The last time I saw someone with that kind of corruption, it was a 9800 Pro with an overheating GPU. You can test that theory by underclocking with ATITool as explained here. If you haven't already, you should also slow down you AGP port and disable fast writes with SMARTGART as shown here.
 
Nope, didn't work. I still exhibit the same symptoms.

Plus, if I try knocking down the clocks as far as 100Mhz below the default speed, the problem looks worse.
 
I tried installing the most recent Omega drivers 2.6.25a. It looks *much* better, but there are still problems.

Also, just for kicks, I tried setting the max GPU and max RAM clocks with the ati tool. About every 3 seconds, it detected an artifact and kicked back the timer.

Are there any completely stable archived omega drivers hosted somewhere? The archive links on the omega site were dead, not to mention I'm not sure which one would be the best choice.
 
I think it's normal for a 9600 Pro to have problems when you underclock the RAM by very much. On my 9700 Pro, I get serious problems if I take it down by too much but I can take my GPU down as far as I want. I've never fiddled with the Omegas but you should be able to get the thing to run with the plain old Catalysts. BTW I tried the archive link at www.omegadrivers.net and it worked fine. Give it another pop.

[edit]: Ok, this is odd...there are strange boxes on the XP loading screen...does this mean there's something else wrong? They are a bit lighter than the colors that they should be.

That makes it sound like you may have a bad bit in your video RAM. A bad bit will change the color. Plus if you look carefully at your ATITool artifact test, you can see that the artifacts are 1 pixel wide and are aligned vertically. I'm not the world's foremost expert on the subject but that sounds like bad bits in your video RAM. I'm not aware of any RAM test programs which can directly check your video RAM.

You might take a careful look at the video card and see if there is any dust or anything else lodged in the pins of the video RAMs. I've occasionally seen people find things stuck in the pins but usually their artifact tests look worse than yours.
 
i own a sapphire atlantis 9600 pro vid card.....do what i did, run ati tool, see highest clock/ram rate keep it there.........413/310 for me, and my games are fine with 5.5 drivers or whatever those recent ones are......for cooling, man sapphire sucks major ramsticks, i got an intell p3 fan ziptied to mine, it works fine with cooling,............get an aftermarket NON VANTEC cooler :p youll be fine

go ahead and overclock it to 413/310, check for errors......

Well, I gave the card a thourough cleaning, first with an air can, then a wipe down with alcohol and the air can again just to be on the safe side. Still no change.
 
Are you able to RMA? and have you tried another vid card in your board? did you overclock it at all?
